Aneurysm of the Ascending Aorta
نویسنده
چکیده
Between January 1983 and March 1988 25 patients with aneurysms of the ascending aorta were operated, out of these only 15 (56%) electively. Acute incompetence (10) were the main indications for operation. Poststenotic aneurysms (2), mycotic (2) and saccular sclerotic (1) aneurysms have also been observed. A rare case of a ruptured ascending aorta due to histologically and serologically proven Brucellosis is presented. Valved conduits were used in 16 patients to reconstruct the aortic root, whereas the remaining patients had different types of supra-coronary reconstruction, i.e. resection with or without graft insertion (6 and 3 respectively). We lost 6 patients out of 25 during or immediately after operation. All of them were operated in an acute state of rupture or dissection with left heart failure, some of them already presenting neurologic deficits preoperatively. Operative and hospital mortality in 14 patients operated electively was 0%. Bentall's operation using valved conduits is a safe operation in elective patients with a reasonable mortality rate worldwide. Clinical follow up should include ultrasound studies of the operative site the remaining aorta, i.v. DSA and CT of NMR scanning in 6-12 month intervals in order to detect late complications as false or dissecting aneurysms.

متن کاملBicuspid Aortic Valve: An Unusual Cause of Aneurysm of Left Coronary Sinus of Valsalva
Bicuspid aortic valve is traditionally considered an innocuous congenital anomaly. Due to a better and widespread availability of non-invasive imaging techniques, it has come to the fore that 30% of these cases develop complications, viz., valve abnormality (aortic regurgitation and stenosis), and aneurysm of aortic root and ascending aorta. Sinus of Valsalva aneurysm is an uncommon complicatio...

Although classic and standard procedure for patients with ascending Aorta aneurysm with Sever aortic Regurgitation is Bentall surgery but in some selected cases this complex surgery was not performed with different reason like advanced age and coexisting comorbidity involving renal and hepatic and lung Function. In our cases 75 years old man with chronic renal failure ( Cr= 3.2 ) who was known ...
متن کاملGiant aneurysm of the ascending aorta secondary to medial agenesis in infancy.
A 14-month-old infant was found to have a giant aneurysm of the ascending aorta at the time he underwent angiography. At the time of autopsy, a saccular chamber was found to take the place of the aorta. Histologic examination of the aneurysm revealed absence of the medial layer. Thus, medial genesis may be another cause of aneurysm of the ascending aorta.

Traumatic false aneurysm of the thoracic aorta is not uncommon. But accordingly this case is unusual in its ten year delay between injury and presentation and treatment. A victim of the Iran-Iraq war is presented with a false aneurysm of the thoracic aorta ten years after a bullet injury to the chest and thoracic spine. Penetrating chest wounds, mechanisms of injury and diagnosis are also ...
متن کاملEquine pericardial roll graft replacement of infected pseudoaneurysm of the ascending aorta
The standard procedure for treating infected aortic aneurysms is to resect the infected aorta, debridement of the surrounding tissue, in situ graft replacement, and omentopexy. However, the question of which graft material is optimal is still a matter of controversy. We recently treated a patient with an infected ascending aortic aneurysm. Because of previous abdominal surgery, the omentum was ...
